Before you can submit this form, you will need to issue a Purchase Order (PO) or credit card in advance and upload your information to the system. Once we receive your payment information, we will contact you within one working day with an RMA number, and an address label will be sent to you.
If you have a valid service contract with GE, please mention your service contract number, which will eliminate the need for a PO.
